Notes:

1. The team manager shall fill this form individually for all the participants including him. 
2. You must login to be able to submit the form. Please contact event coordinator to get login credentials by email. 
3. Each team will have maximum of 11 players including a team manager.
4. The submissions can be saved and edited till 11-Feb-2023 using a List of Participants link given on this page in the side menu.
5. The names entered will printed as it is in the Certificate.
